Browsing the Atlantic top division league table I clicked on Pivacs Unknown 23, went to their games played, and in some games it shows the team name Dulka Prague in place of Pivacs.

Yes that's because before the season kicks off the fixtures for the upcoming season are pre set in the game engine. The option to change your team name is still open after the fixtures have been decided. This is why all Tom's league matches show as Dukla Prague Away Kit because thats what his team was named when the fixtures where decided. Tom's friendly matches and cup matches will be under his teams new name Pivac's unknown 23 as these fixtures are only decided after the season starts.
